Patient Resources

Where do I start?

Reaching us is fairly simple. If you have reached this page you can either call or email to schedule an appointment or simply do it online.

Do I need to see a script from a doctor before coming for physical therapy?

No. The  District of Columbia is a direct access state meaning that you can see a therapist without seeing a doctor and most health insurances reimburse for them as well. The only exception is Medicare, where a script is necessary.

What should I bring with me?

For your first visit you will need your insurance card,  photo ID, and any paperwork that your physician  has given you to take to physical therapy. You can go to the forms tab on our page and fill out forms before your visit or come 15 min prior to your appointment time to finish paper work,

What should I expect on my first visit?

Your first visit will consist of an evaluation which includes establishing your goals and a treatment session. A typical evaluation lasts about 60 min. Loose fitting clothes are preferred to make examination and treatment easier. You are welcome to bring a change of clothes.

How long will each appointment last?

Initial evaluations are scheduled for 60 min, and the follow up sessions are 30 min each.

What type of insurance do you accept?

Capitol Physical Therapy accepts most major insurances . Our administrative personnel will verify your insurance coverage and discuss it with you prior to your initial visit.

How much will each visit cost me?

It will vary depending on your insurance. Please call for current cash rates if you do not have insurance.

Can I fill in my information beforehand so I don’t have to wait when I visit?

Please visit our Forms page. It has all the forms that you can either fill out online or print, fill and bring with you.
